The New York State Education Department requires a Health Examination (on the approved NYSED Student Health Examination Form for School) for all students entering the district for the first time and when entering Pre-K or Kindergarten, 1st grade, 3rd grade, 5th grade, 7th grade, 9th grade and 11th grade.
